The Fund for Public Health in New York City (FPHNYC), in partnership with the New York City Department of Health and Mental Hygiene (DOHMH), is seeking a Community Health Worker (CHW) to support the Health & Affordability Corps (H&AC). This initiative focuses on connecting New Yorkers to health coverage, public benefits, and essential social services. The CHW will act as a trusted liaison, helping individuals access resources that support their health, well-being, and economic stability, while promoting health equity across New York City. The Health & Affordability Corps (H&AC) aims to improve health equity by assisting New Yorkers in accessing and maintaining essential healthcare coverage and public benefits. The program measures its impact by increasing healthcare access, reducing barriers to critical services, and enhancing financial stability for individuals and families. Through direct outreach, education, enrollment support, and community engagement, H&AC ensures New Yorkers receive the resources needed for their health and well-being.
