The Community Health Worker is part of Rural Health Connect South Jersey (RHCSJ), a community-based prevention program with the goal of identifying and engaging uninsured and underinsured rural residents of Atlantic, Cape May, Cumberland and Salem Counties. The RHCSJ team will conduct health screenings and connect individuals and families to preventative physical health, behavioral health, and family support services. Through community outreach and engagement, the program intends to improve early identification of health risks through screenings, increase access to services, and reduce unmet health needs among this population. As an integrated health organization, Acenda values the use of evidence-based practice or EBP's.
