About The Position

Engages and provides peer support to group health members. Serves as a role model in maximizing wellness and independence. Collaborates with clinical and non-clinical team members to ensure a culture of support is reflected in members’ treatment. This role supports Albany, Central New York, and Rome/Utica regions and their respective surrounding counties across New York State with a Hybrid work model. Applicant must live within the health plan service areas or surrounding counties and will support a hybrid work model with remote work and travel to plan member's residences.

Requirements

  • Associates degree in human services, Licensed Practical Nurse, or equivalent of 1-3 years of relevant experience, with high school diploma or equivalency required.
  • Strong computer literacy and comfort, ability, and desire to advance technically and understand industry standards.
  • Proven analytical and problem-solving skills to identify potential issues and participate in their resolution.
  • Proficiency with computer systems, working knowledge of Microsoft Office, Internet, and various software applications.
  • Valid NYS driving license required with own reliable transportation required.
  • Ability to sit or stand at a desk 90% of the day.
  • Travel throughout NYS.
  • Visual/hearing ability sufficient to comprehend written or verbal communication.
  • Ability to express self verbally and in writing.
  • Ability to use telephone and communicate over the phone.
  • Frequently uses a laptop computer to enter orders and obtain product information.

Responsibilities

  • Travels to members’ homes to evaluate their plan of care and ongoing needs for community health services and support at least every six months or as directed by Clinical Team.
  • Collaborates with clinical and non-clinical team members to ensure a culture of support in reviewing and updating the members’ care plans.
  • Provides support to health plan members, including orientation to the health plan, and the review of members’ care plan including services and supports.
  • Reports on members’ current level of care needs in the community to clinical team members.
  • Refers appropriate questions and issues to clinical and non-clinical team members.
  • Conducts outreach to members who are missing monthly contacts.
  • Assist with entitlement initiatives such as Medicaid benefits, spend-down, heating and food stamp assistance, and housing.
  • Communicate with internal and external stakeholders in a timely and courteous manner.
  • Participate in departmental meetings and attend all scheduled staff meetings and staff training as applicable.
  • Perform related duties as required.
