About The Position

A Non-Merit vacancy exists in Clinical Services/Maternal Child Health, Department of Health and Human Services. Non-Merit positions are not classified within the Baltimore County Government Classification and Compensation Plan. A list of eligible applicants will be established based on the examination as outlined below. Requirements

Possession of a high school diploma or appropriate equivalent.
At least one year experience working with the public.
Knowledge of English grammar.
Knowledge of and sensitivity to cultural influences.
Ability to learn and follow county and departmental rules, regulations, policies, and procedures.
Ability to establish effective working relationships with clients, families, and public and private agencies.
Ability to communicate effectively with individuals and groups.
Ability to work flexible hours.

Ability to coordinate activities.
Ability to maintain inventory of supplies.
Possession of a valid driver's license equivalent to a noncommercial, class C Maryland driver's license is required at the time of appointment.

Responsibilities

Performs routine community health worker duties for the Babies Born Healthy Program.
Provides care coordination and case management.
Conducts virtual and in-home interviews (as needed) and virtual telehealth visits to pregnant and post-partum women.
Provides care coordination and perinatal navigation services.
Connects women to resources, information and distributes supplies.
Provides care coordination, health education, identifies resources, and conducts telephone and in-home perinatal navigation services for pregnant and postpartum women in the program.
Conducts interviews and performs screenings for high-risk pregnant and postpartum women to identify needs, link to resources in Baltimore County and provide education on prenatal and postpartum topics.
Coordinates with community partners and agencies for available inter-agency community programs and services.
Assists and links clients and families with resources.
Completes documentation, encounter forms and data tracking.
Maintains cooperative and liaison relationships with public and private agencies.
Performs other related duties as required.
