Shenandoah Community Health (SCH) is a Federally Qualified Health Center dedicated to bridging gaps in healthcare access by addressing challenges such as cost, transportation, and language barriers. SCH offers a comprehensive range of services including primary care for adults and children, women's health, behavioral health, and oral health, along with on-site pharmacy, labs, and radiology. Requirements
- High school diploma or GED
- Strong communication and relationship-building skills
Nice To Haves
- Knowledge of community resources preferred
- Bilingual (English/Spanish) preferred
- CHW certification or willingness to obtain certification preferred
Responsibilities
- Connect patients to healthcare and community resources
- Help individuals overcome barriers to care
- Support chronic disease management and healthy lifestyles
- Provide health education and advocacy
- Work alongside providers, nurses, and care coordinators
- Participate in community outreach activities
