The Community Health Worker will provide individualized companion support and coordination of services to older adults who participate in the LSS Healthy Transitions and Friendly Helper programs through healthcare partnerships with Lutheran Social Service of Minnesota. The position is responsible for supporting the older adult’s transition from hospital to home by increasing the participant’s access to essential community resources, providing education and support that increases stabilization, as well as reducing additional emergency-room visits and hospitalizations. The position is also responsible for engaging clients in service by providing direct care to increase socialization and connect to community and resources in an assigned geographic area. The responsibilities include program outreach, friendly visits (telephonic and in-person), light household chores, personal errands such as grocery shopping and transportation. This position is also responsible for documenting direct care visits of members in service, assuring compliance with program regulations, and managing relationships with multiple stakeholders in the community.
