About The Position

Ozarks Community Hospital (OCH), located in Springfield, MO, has an opening for a Community Health Worker (CHW) within its Medical Home department. The CHW will be an integral part of the client resource team, helping to bridge patients to resources to reduce social determinants of health that impact clients' health and well-being. This role involves providing informal coaching, counseling, social support, culturally appropriate health education, and basic screening services. The CHW is expected to perform duties consistent with the Standards of Practice, goals, and philosophy of Ozarks Community Hospital and the Employee Code of Conduct. Requirements

  • High school diploma or equivalent
  • Must obtain CHW certification through an approved curriculum within 12 months of hire date
  • Valid Driver's License required
  • Must have a valid Class E license upon hire or Class E license must be obtained during orientation period
  • Current BLS certification upon hire or BLS certification must be obtained during orientation period or at the next available scheduled class
  • Throughout employment, current BLS certification must be maintained by renewal during renewal period or at next available scheduled class
