About The Position

We are a private, not-for-profit organization that coordinates public health grants for a 26-county region in central PA, seeking a Community Health Worker (CHW). This is a full-time hybrid position located within our Camp Hill location, balanced with opportunities for work-from-home days. The Community Health Worker (CHW) will provide housing support services and other case management duties as assigned to clients with lower incomes in their communities. CHW will work collaboratively with all applicable internal and external stakeholders' staff and partners to identify needs and connect clients to housing, healthcare and social services.

Requirements

High School Diploma or GED (General Education Diploma)
CHW Certification (training will be provided upon hire)
Strong verbal and written communication skills
Strong time management and prioritization skills
Computer literacy including Microsoft 365
Ability to work with cross-disciplinary teams of diverse individuals, including those with lived experience
Reliable transportation
Valid PA driver's license
Up-to-date vaccinated status (influenza and COVID-19, and potentially others based on public health needs)
PA State Police Criminal Record Check (PATCH)
FBI Criminal Record Check, from fingerprinting
PA Child Abuse History Clearance (PACA)
