About The Position

Community Health Worker (CHW) Make a Difference in Your Community Boston Mountain Rural Health Center (BMRHC) is seeking a compassionate, motivated Community Health Worker (CHW) to join our team. In this role, you'll serve as a trusted advocate and resource for patients, helping them overcome barriers to care, access financial assistance programs, and connect with services that improve their health and quality of life. Position Summary The Community Health Worker serves as a vital link between patients, healthcare providers, and community resources. This role focuses on helping patients navigate healthcare and financial assistance programs, including insurance enrollment, sliding fee applications, and payment plans. The CHW also provides education, outreach, advocacy, and support to help patients access the care and services they need to thrive.

Requirements

  • High school diploma or equivalent.
  • Active BLS certification.
  • Certified Community Health Worker (CHW), eligible for Arkansas CHW certification through the Experiential Track, or ability to obtain certification within one year of hire.

Nice To Haves

  • Some college coursework and/or healthcare experience.
  • Knowledge of insurance programs, financial assistance processes, and community resources.
  • Experience working with diverse populations.
  • Bilingual skills a plus (English/Spanish or other languages).
  • Experience using electronic health records and Microsoft Office or Google Workspace.

Responsibilities

  • Guide patients and families through financial assistance programs, including Medicaid, Medicare, Marketplace plans, and BMRHC's sliding fee program.
  • Work closely with front office, billing, and finance teams to develop manageable payment arrangements.
  • Connect patients with community resources related to food, housing, transportation, employment, and other social needs.
  • Provide culturally sensitive health education and support.
  • Serve as a bridge between patients and healthcare providers to improve communication and care coordination.
  • Identify barriers to care and help patients find practical solutions.
  • Participate in community outreach events, health promotion activities, and quality improvement initiatives.
  • Advocate for equitable access to healthcare and community resources.
  • Perform other duties as assigned.
