About The Position

This role focuses on addressing the healthcare needs of youth in Lynn by promoting, facilitating, and improving access to ongoing primary and behavioral health care. The Community Health Worker will collaborate closely with the nurse practitioner, behavioral health provider, and other members of the LCHC team to provide education, advocacy, and coaching to youth requiring additional support for medical, social, and psychological needs. The position also involves assisting patients and/or families in navigating healthcare and social service systems, arranging transportation, and accompanying patients to appointments as needed. Additionally, the role includes participating in community outreach efforts and conducting engagement activities to support patient continuity of care, including re-engaging patients who miss appointments or do not follow up on treatment.

Requirements

  • Three or more years experience as a community health worker or case manager in a community health setting; and/or related experience in health/human services, social services or advocacy.
  • Experience in outreach within mental health, community-based treatments or medical comorbidity.
  • Strong organizational, interpersonal, attention to detail, time management, multi-tasking, and problem-solving skills.
  • Willingness to openly address and acknowledge issues of addiction, substance use and mental illness.
  • Must have a valid driver’s license and transportation.

Nice To Haves

  • Bachelor’s degree in health sciences, social work, sociology, human services, or related field preferred.

Responsibilities

  • Educate, advocate and coach Lynn youth who require additional support to meaningfully address medical, social, and psychological needs.
