About The Position

Under the direct supervision of the ECM Supervisor, the Community Health Worker (CHW) is a person with direct knowledge of the local community who functions as a “guide” and aids community members in “navigating” the health care system and accessing related social and financial services to improve health outcomes. The CHW, alongside a Lead Care Manager (LCM), performs a variety of health promotion, clerical, and medical support services within the clinic setting and in the community. A primary function of the CHW is to outreach and enroll eligible members into United Health Centers of the San Joaquin Valley’s ECM Program and maintain communication between members, their families, physicians, and the health care and social system(s) to help facilitate the care of individuals. NOTE - Travel is required for this position and Bi-lingual in English/Spanish is required for this position.

Requirements

  • Medical Assistant certificate (Certified or Registered Medical Assistants preferred) OR Community Health Worker (CHW) experience OR Medical Reception Experience.
  • At least 1 year of previous experience in a health care setting providing evidence-based health education and training to patients to avoid illness, injury or lessen its effects, preferred.
  • Knowledge of medical terminology and chronic health conditions preferred.
  • High school Diploma or GED required.
  • Valid driver's license
  • 1 year of previous experience in a health care setting, performing clerical tasks and providing evidence-based health education and training to patients to avoid illness, injury or lessen its effects, preferred.
  • Bilingual (English/Spanish) required.
  • Able to quickly build and maintain rapport with staff of differing backgrounds; team player
  • Customer-service oriented
  • Direct knowledge of the local community.
  • Knowledge of basic healthcare billing functions
  • Ability to work effectively with diverse populations providing advocacy and support.

Responsibilities

  • Conducts outreach and enrollment activities as directed to engage members identified as being ECM eligible.
  • Outreach activities may occur onsite and in the community per established field outreach protocols.
  • Performs billing functions with accuracy, such as insurance verification and claims submission to support accurate service reimbursement.
  • Works with care team staff to identify potential members to outreach and enroll into the ECM program.
  • Collaborates with multidisciplinary teams to communicate, coordinate, and provide patient care.
  • Performs, as needed, member scheduling, transportation scheduling/assistance, program eligibility screening, broken appointment follow-ups, etc.
  • Utilizes language and cultural awareness skills to outreach to members and community and present the ECM Program in an acceptable manner.
  • Able to balance priorities and maintain outreach and enrollment goals.
  • Offers services where ECM enrolled members live, seek care, or find most easily accessible and within health plan guidelines.
  • Connects with ECM eligible members via texting campaigns, letters, phone, or in-person to facilitate enrollment into ECM.
  • Accompanies ECM enrolled members to office visits, as needed and according to health plan guidelines.
  • Utilizes EHR and other electronic tracking systems to coordinate services and input data for reporting.
  • Maintain accurate documentation of patient interactions and services provided.
  • Attends required ECM trainings and meetings.
  • Assists with the required ECM reports, data entry, and other internal reports.
  • Responsible for coordinating with individuals and/or entities to screen for program eligibility and ensure a seamless experience for members and non-duplication of services.
  • Use motivational interviewing, self-management support, trauma-informed care, and harm-reduction approaches.
  • Other duties as assigned.
