The Community Translational Research Institute (CTRI) is dedicated to improving population health through the translation of prevention science into community-based programs and sustained practice and policy. CTRI partners include leaders from Claremont Graduate University, County of Riverside, Inland Empire Health Plan, and University of California Riverside School of Medicine. The Community Health Worker (CHW) serves as a frontline advocate for population health and engagement initiatives. This role involves outreach, screenings, and care navigation for vulnerable populations, such as Medi-Cal members and refugees, to address health equity gaps. The CHW builds community trust to facilitate service enrollment and provides culturally responsive health education through various grant-funded programs. This position supports services under the Inland Empire Health Plan (IEHP) Participating Provider Agreement for Community Health Worker Services, requiring adherence to IEHP guidelines, documentation standards, and regulatory requirements. This is a full-time (40 hours per week) benefited position provided by Heluna Health.
