Community Care of North Carolina (CCNC) is transforming healthcare across North Carolina by utilizing statewide data and predictive analytics. Community-based care-managers collaborate with local physicians and diverse health professional teams to create comprehensive care plans. These plans connect individuals with local resources, aiming to increase equity and access to high-quality care. The mission of CCNC is to improve the health and quality of life for all North Carolinians by building and supporting better community-based healthcare delivery systems. The Community Health Worker (CHW) is a non-clinician role that functions as part of the primary care health team. This role supports a multi-disciplinary approach to patient care by delivering comprehensive, coordinated CHW interventions. Key performance indicators include community outreach and education, completing Health Risk screenings, and initiating referrals to Care Management. CHWs may directly assist members in their communities to improve their health outcomes. They also contribute to designing and implementing systems that ensure members have access to knowledge, resources, and support from the Care Team.
