About The Position

EAC Network, a leading not-for-profit social service agency, is seeking a dedicated Community Health Worker to join the Social Care Network Program. This role focuses on engaging individuals and the community to assess their qualifications for enrollment into the Social Care Network (SCN). The Community Health Worker will conduct screenings, eligibility assessments, and referrals to appropriate services, as well as promote the program at various events.

Requirements

  • Associate’s Degree in the human services field or equivalent plus one year of related experience required.
  • Must have professional appearance, excellent organizational and time management skills, strong communication and public speaking skills and the ability to work independently.
  • Must have valid NYS (New York State) Driver’s License and be willing to transport clients to appointments using own vehicle.
  • Must provide proof of valid NYS car insurance and Defensive Driving Certificate within 30 days of hire.
  • Valid registration, inspection and Automobile Insurance Liability of a recommended minimum of 100,000/300,000 will also be evaluated upon hire.
  • Must be proficient in computer applications or other automated systems such as spreadsheets, Microsoft Office applications, calendar, email and/or database software necessary to perform work assignments.
  • Must have ability to rapidly navigate workflows within a technology platform.

Responsibilities

  • Engage individuals eligible for services through the Social Care Network through various methods (phone, face to face, etc.) in order to enroll them into the program.
  • Conduct outreach to Medicaid populations residing in Nassau and Suffolk counties and utilize a standardized screening tool to assess their health-related social needs for the SCN.
  • Conduct progressive outreach attempts for all potential clients at a minimum on a weekly basis.
  • Assess client eligibility for a range of services through the SCN and connect them to any health-related social needs services they qualify for.
  • Maintain all necessary documentation in the Electronic Health Record (EHR) in a timely manner.
  • Work collaboratively with Division Director and other team members regarding any client needs.
  • Participate in supervision to address concerns/issues and improve skill development.
  • Participate in network partner engagement meetings, staff meetings, and planning meetings, among others.
  • Travel to Nassau and Suffolk and participate in community events to promote the program to potential eligible clients.
  • Develop and maintain an in-depth knowledge and understanding of the range of services available throughout the SCN.
  • Serve as a liaison between participants, healthcare providers, and community-based organizations.
  • Carefully document outreach, screening and all referral information within the SCN platforms.
  • Perform all other relevant duties as determined by Supervision and EAC NETWORK Administration.
