Requirements

  • High School Diploma or its equivalent.
  • Successful completion of a Community Health Worker formal training program or Associates Degree, within one year of date of hire.
  • Lived Experience with addiction treatment (individuals that have been successful in the recovery process and help others who are experiencing similar situations) or Peer Recovery Support Specialist.
  • Ability to be insured on the commercial auto insurance policy based on driving record including active driver’s license and personal automobile insurance and reliable transportation.
  • Employment offers conditional upon completion of a background screen including fingerprinting and a drug screen.
  • Within the first 90 days of employment: BLS and completion of health screenings as outlined in the credentialing policy are required for successful completion of orientation.

Responsibilities

  • Conduct required monthly home visits to provide clients with learning opportunities, guidance, and resource connections to support their recovery.
  • Complete and maintain accurate written and electronic database records in accordance with ODH and/or grant funder standards.
  • Coordinate and/or conduct initial and ongoing screening/assessments of clients; determine concerns, priorities, and resources for each client and assist them with creating goals. This includes the required GPRA interviews and documentation.
  • Adheres to timelines mandated for enrollment, assessments, and screenings outlined by the Ohio Department of Health and the community Pathways Hub and grant entity.
  • Work alongside clients to connect to services in a way that promote self-sufficiency.
  • Facilitate communication and client involvement in referrals to other agencies and maintaining records and other documentation related to the referrals.
  • Plan and organize own work to meet priorities and deadlines (daily, weekly, monthly, quarterly, yearly)
