About The Position

The Community Health Worker (CHW) – Quality I will assist in the overall goal of increasing patient connections to services for: preventative cancer screenings, chronic diseases, and mental health, medical, oral health services provided at Greater Portland Health. The CHW – Quality I should approach work with empathy and understanding and provide appropriate navigation to connect patients to cancer screenings.

Requirements

  • Level 3 English required.
  • High school diploma or GED equivalent required
  • Knowledge of Microsoft Word and Excel required
  • Valid Maine Driver's License (we will conduct a review of your driving history)

Nice To Haves

  • Experience with electronic health records desired

Responsibilities

  • Provide appropriate navigation to connect patients to cancer screenings.
  • Drive the mobile medical unit and its team into the community.
