About The Position

The Community Health Worker (LTSS) will manage community health by establishing and maintaining trusting relationships with individuals, families, and providers to promote health, recovery, resiliency, and wellness. This role involves advocating for and supporting members and their families by prioritizing their needs and preferences, ensuring they receive appropriate care and services promptly. The position requires coordinating and collaborating with licensed clinicians and Health Management programs, adjusting workflows as necessary to provide optimal care. Effective communication techniques, including motivational interviewing, will be used to encourage members to attend appointments, close care gaps, and utilize health services. The role also involves delivering comprehensive education, resources, and referrals to community health-related services, and guiding community members through complex health cases. Responsibilities may include home visits, accompanying individuals to appointments, and supporting community events, all while exercising sound professional judgment and adhering to scope and licensure boundaries.

Requirements

  • Associate's degree and three years relevant healthcare/social services experience; or equivalent education/work experience.
  • Fieldwork experience such as home visits, conducting health screenings in the community, and/or navigating social services (like food, housing).
  • Excellent communication skills, both written and verbal.
  • Basic working knowledge of Microsoft Office applications, including but not limited to Word, Excel, and Outlook.
  • Must have reliable transportation for in-person meetings with members at home or other locations.

Responsibilities

  • Manage community health by establishing and maintaining trusting relationships with individuals, families, and providers to promote health, recovery, resiliency, and wellness.
  • Advocate and support members and their families by prioritizing their needs and preferences, making sure they receive appropriate care and services promptly.
  • Coordinate and collaborate with licensed clinicians and/or Health Management programs, as needed.
  • Adjust workflows when necessary to provide optimal care for each situation or individual.
  • Use effective communication techniques, including motivational interviewing, to encourage members to attend regular provider appointments, close open care gaps, and utilize medical services that support their health.
  • Engage with members and their families to discuss major health concerns and explore solutions to obstacles impacting service delivery, member satisfaction, cost, and community health.
  • Deliver comprehensive education, resources, referrals, and connections to health-related services within the community.
  • Facilitate the introduction of new or underutilized programs and services to community members.
  • Provide guidance to community members facing complex health cases in navigating healthcare systems.
  • Conduct home visits, accompany individuals to appointments, and support community events as needed.
  • Exercise sound professional judgment and adhere strictly to scope and licensure boundaries to ensure actions are taken in the best interest of the community member(s).
  • Provide culturally appropriate health education and instructions on using existing health and social services, presenting information clearly and effectively.
  • Perform all other miscellaneous responsibilities and duties as assigned or directed.
