The community health worker position plays a key role in implementing services, projects, and programs that adhere to state and federal regulations, support public health efforts, and work alongside community partners to improve health services. As part of Crook County Health & Human Services, this position will collaborate on various public health initiatives that align with the department's mission to provide fair opportunities for those in our community to achieve their full health potential. This includes implementing data-driven, developmentally appropriate, culturally aware, trauma-informed, and sustainable approaches. This position directly supports the successful implementation of core services offered at Crook County Health and Human Services in an administrative & prevention capacity. This position will liaise between two main core services: Administrative and Prevention & Health Promotion. Administrative: This position will serve as a receptionist for clinical services and work within a variety of reporting systems and Electronic Health Records to perform chart reviews, appointment scheduling, and documentation. Prevention: This position will work closely with the Prevention and Health Promotion Team to collaborate on, and complete initiatives outlined in the Tobacco Prevention & Education Promotion (TPEP) workplan. This position is considered confidential and sensitive requiring attention to detail, reliability, and the ability to work collaboratively and independently with various teams and co-workers. This position requires excellent customer service and the ability to assess an individual's need and connect them with services internally and externally. This position requires a self-motivated and proactive thinker who responds well to constructive feedback.
