About The Position

This position carries out a variety of health service duties involving communication, educational, and training activities under the supervision of, or after consultation with, one or more health specialists.

Requirements

• An Associate of Arts degree from a community college or a satisfactory combination of education and experience equivalent to two years of college work above high school.

Responsibilities

• Arranges meetings and meets with groups in the community in need of health services; explains programs available and official and other community resources.
• Participates in and promotes health education activities within the community.
• Refers individuals and groups to health specialists within a local health department or to other community resources.
• Provides ongoing individualized case management services.
• Identifies health services needs of individuals and groups within minority and disadvantaged groups.
• Oversees and coordinates volunteer advocacy activities.
• Participates in planning and carries out program activities in cooperation with one or more health specialists.
• Prepares reports; participates in oral and written plans, analyses, and evaluations of health services.
• Performs other work as required.
