About The Position

The University of Arkansas for Medical Sciences (UAMS) has a unique combination of education, research, and clinical programs that encourages and supports teamwork and diversity. The Community Health Worker (CHW) will present community education programs, participate in health fairs and other health promotion activities, assist with maintaining an up-to-date referral resource list for fifteen counties, work with other community service providers, assist with maintaining the Center on Aging’s program files and database as well as other administrative duties, and provide opportunities for exercise programs for older adults in the assigned counties in person, hybrid, and virtually. Must have the willingness to work with multi-disciplinary teams internally and externally in various communities in an integrated model of health and social services care delivery team, while striving to improve health outcomes for older adults.

Requirements

  • High School diploma/GED plus two (2) years of general experience in community outreach, education, case management, or administration experience required.
  • Must enroll in a certified CHW Training Program within 45 days of employment.
  • Must be proficient with Microsoft Office.
  • Persons must have proof of legal authority to work in the United States on the first day of employment.

Responsibilities

  • Plans and implements various educational and exercise programs on aging topics for families, patients, and the general community.
  • Assists communities in building capacity by identifying resources.
  • Coordinates service and support providers, linking groups or systems that provide synergistic support, and implementing advocacy strategies to address unmet needs.
  • Maintains relationships and develops joint programming with providers and community leaders in the counties served.
  • Maintains documentation of all programs via evaluation, data for monthly, quarterly, and yearly reports, correctly accesses and updates program files and database, and serves in an administrative role via appropriate procurement standards.
  • Performs other duties as assigned.
