About The Position

We’re pleased to announce an opportunity for the position of Community Health Worker – Foundational Community Supports within the Community Based Teams Department. This role responds to clients in need of services and supports, providing aid to help qualified clients obtain and maintain safe and stable housing. The Community Health Worker works closely with the Program Development Manager to ensure project success and effective communication amongst all necessary program components. They maintain a timely and accurate record of responsibilities and completed tasks, conduct housing supportive work with clients, and help to address social determinants and unmet social needs.

Requirements

  • High school diploma or GED and related experience working with individuals on care plans or with community partners OR Any combination of education and experience that would provide the applicant with the desired skills, knowledge, and ability required to perform the job.
  • Acceptable completion of a criminal history background check.
  • Acceptable completion of Community Health Worker’s Certification program within 6 months of employment.
  • Valid driver’s license in state of residence.
  • Auto insurance in the amount required by the State of Washington.
  • Access to reliable transportation.
  • Resume Required

Responsibilities

  • Conduct in-person visits with clients across designated service areas, requiring frequent travel by vehicle. This includes driving to client homes, community settings, and partner agencies to perform assessments, deliver case management services, and facilitate referrals.
  • Complete the initial interviews with the client and their family to assess the client's eligibility and need for housing and other related services.
  • Collaborate with internal teams and external partners to address clients’ social determinants of health and unmet social needs.
  • Educate client and their family about available community resources and work to connect them to local services.
  • Keep up to date and accurate documentation utilizing a variety of software to support the program.
  • Performs other duties as assigned.
