Requirements

  • High School Diploma or equivalent required.
  • Must obtain a Community Health Worker certification within 1 year of hire, in accordance with state and organizational standards. Ongoing continuing education may be required to maintain certification.
  • 0-3 years of experience required.
  • Requires basic computer skills.
  • Requires knowledge of medical, community, and social resources.

Responsibilities

  • Provide outreach, education, and ongoing support to pregnant individuals and families throughout pregnancy, labor, and the postpartum period.
  • Serve as a trusted advocate and bridge between the community and healthcare, government, and social service systems.
  • Assist clients in identifying and overcoming barriers to accessing healthcare and community resources.
  • Connect individuals and families to essential services, including: Prenatal and postpartum healthcare, Health insurance enrollment and navigation, Food assistance programs, Housing resources, Transportation services, Childcare and family support programs, Community-based health and wellness services.
  • Conduct assessments to identify client needs and develop appropriate resource and referral plans.
  • Provide culturally responsive health education related to prenatal care, maternal health, infant care, and healthy lifestyles.
  • Support clients in navigating healthcare systems, scheduling appointments, and understanding recommended care plans.
  • Offer informal counseling, encouragement, emotional support, and follow-up services to clients and families.
  • Advocate for clients to ensure access to quality healthcare and supportive services.
  • Maintain regular communication with clients to monitor progress and address emerging needs.
  • Collaborate with healthcare providers, community organizations, and social service agencies to coordinate care and services.
  • Facilitate referrals and ensure successful connections to community resources and support networks.
  • Maintain accurate documentation of client interactions, referrals, services provided, and outcomes.
  • Participate in community outreach events, educational programs, and health promotion activities.
  • Promote positive maternal and infant health outcomes through education, advocacy, and resource coordination.
  • Support family well-being by encouraging self-sufficiency, healthy behaviors, and engagement with available community resources.
