About The Position

This position works as a member of the Accesshealth Horry team with high-need uninsured adults in underserved areas of Horry County for the purpose of helping individuals obtain the community health care and resources they need. The CHW is a “front-line” public health professional who lives and works in challenged communities and populations and must spend at least 60% of their time in the community. The CHW will help individuals navigate both the health and human services system; provide culturally appropriate health education; help build community capacity (for more resources) and provide needed follow-up. The CHW is expected to provide support, encouragement, and guidance to individuals who need the most assistance. The goal of the CHW is to be a positive change-agent and help reduce health disparities in underserved communities.

Requirements

  • High School Diploma or GED.
  • Must be familiar with Horry County and have “lived” experience in the area.
  • Strong people and communication skills, both verbally and in writing.
  • Demonstrates a strong and positive desire to help others.
  • Ability to gather data, collect and compile information, and prepare reports.
  • Be a self-starter and able to work independently.
  • Must have a valid driver’s license and provide own transportation.

Nice To Haves

  • Prefer someone who is already connected to the community through prior work experience and/or volunteer work, involvement with charitable organizations, faith-based communities, hobby groups, etc.
  • Ability to speak Spanish preferred, but not required.
  • Ability to work some weekends.

Responsibilities

  • Assists individuals with health education and social determinants of health.
  • Advocates for the needs of individuals and populations in the community.
  • Provides informal counseling and guidance on health-related issues.
  • Educates individuals on how to use community health and social service systems.
  • Conducts home visits to provide education, assessment, and social support.
  • Helps identify community resources in underserved areas in the county.
  • Educates the health and social service providers in the community about the needs of individuals who receive their care.
  • Helps build relationships with community partners and expand the network of available resources.
  • Assists individuals with self-management of chronic health conditions and medication adherence.
  • Helps individuals set measurable health improvement and self-management goals.
  • Helps build trusting relationships (individual and community) and partnerships.
  • Collects data on community health needs and reports any barriers to services or system related challenges to leadership.
  • Serves on community coalition work groups and attends meetings related to community resources for the uninsured.
  • Documents all contacts with assigned patients in a timely manner.
