About The Position

FuntoNetwork is seeking a Community Health Team Tracking Tools Implementation Lead for an active RFP opportunity: Vermont DVHA NOA 03410-240-26 - Rural Health Data and Analytics Infrastructure Grant. DVHA is offering a Rural Health Transformation Fund grant for eCQM calculation tools, community health team tracking tools, and primary-care data analytics infrastructure. This role involves coordinating the implementation and adoption of community health team tracking tools, documentation, training, and stakeholder readiness. The lead will also review RFP scope, source posting, supporting documents, deadlines, and client requirements, and coordinate with FuntoNetwork and related consultants to support a complete, professional response. Preparation of documentation, analysis, recommendations, deliverables, or technical inputs related to this role, as well as clear communication with technical and non-technical stakeholders, and support for quality control, status updates, and final submission readiness are also core responsibilities.

Requirements
