Job Overview The Community Health Supervisor is responsible for overseeing the Community Health Worker (CHW) team and their functions. The Supervisor provides supervision, coaching, and direct support to the CHW team, and monitors the progress of the program/panel management. The Supervisor provides leadership to the team through training, mentoring, professional development, management of disciplinary actions, annual evaluations, case discussion, reviews, schedule management, and more. Other responsibilities of the Supervisor are LEM system updates, identification of resources within the organization by fostering cross-functional relationships with other internal services, collaborates in updating policies and procedures, monitors the team’s time and attendance while ensuring coverage, leads interviews, and identifies/escalates training needs. The Supervisor demonstrates an advanced level of communication and interdisciplinary collaboration, professionalism, and systems-based thinking with an emphasis on the importance of patient-centered care, education, and organizational involvement to meet the team’s identified goals. The Supervisor effectively communicates feedback and provides ongoing coaching and mentoring to staff while supporting a learning environment to advance team skills and promotes professional development while working closely with Lead CHW.

Requirements

  • High School Diploma or GED is required, AA/AS preferred.
  • Minimum 3 years of experience directly working in the health care environment or a community outreach setting required.
  • Bilingual (Spanish/English) or another secondary language required due to population being served.

Responsibilities
