About The Position

The main purpose of this position is to conduct evaluations of referred and identified clients from the senior service program. For the AERS and Senior Services clients, the purpose being to help those individuals reside in the least restrictive setting with the appropriate and necessary services while functioning at the highest possible level of independence and personal well being. These evaluations include STEPS, interRAI and PASRR assessments, and other health assessments and screenings. For the Mobile Integrated Health Program, the purpose being to provide access to resources in order to reduce non-emergent 911 calls and ED utilization. For all populations, provide case management and care coordination service to assist the clients receive the recommended services. Act as Nurse Monitor, as needed, for clients receiving Community First Choice (CFC) services, Community Personal Assistance Services (SPAS), or are enrolled in the Community Options (CO) waiver programs.

Requirements

  • A Bachelor’s degree in nursing from an accredited college or university.
  • One year of experience as a Community Health Nurse.
  • Candidates for positions in this classification must possess a current license as a registered nurse from the Maryland State Board of Nursing, 4140 Patterson Avenue, Baltimore, Maryland 21215.
  • Employees in this classification may be assigned duties which require the operation of a motor vehicle. Employees assigned such duties will be required to possess a motor vehicle operator’s license valid in the State of Maryland.
  • Employees in this classification will be required to provide the facility with a telephone number at which they can be reached.
