About The Position

JOB PURPOSE: The Community Health Supervisor (CHS) collaborates with clinic leadership and the Arizona Alliance for Community Health Centers for Social Determinants of Health to develop work flow, policies and procedures for employees who are working with Social Determinants of Health. The Community Health Supervisor serves as the business development liaison, consultant and patient advocate for El Rio Health by leading and monitoring grant funded projects and research pertaining to Social Determinants of Health (SDOH). Essential Job Functions: Responsible for leading and coordinating all Community Health Advisor functional, technical, and operational activities. Demonstrates an understanding of and proficiency with the application of all Joint Commission Accreditation standards and reporting requirements applicable to a Community Health Advisor Supervisor. Supervises and oversees Community Health Advisor collaboration in pilot programs and grant funded projects.Supervises community Health Advisor policies and procedures with Social Determinant of Health and participates in PRAPARE train the trainer with The Arizona Alliance for Community Health Centers for Social Determinants of Health. Assists the manager in organizing, planning and implementing strategy Maintains staff by recruiting, selecting, orientating and training employees; provides personal growth opportunities to staff; Controls expenses by gathering and submitting budget information, scheduling expenditures, monitoring variances, and implementing corrective actions.Supervises, at the direction of the Eligibility Manager, the implementation of strategic department objectives, policies, and procedures; effectively interpreting and implementing policies, procedures, and best practices supporting successful service, deliverables, and/or outcomes Completes operations by: Organizing workflow to accomplish established objectives Developing schedules Delegating responsibility Evaluating subordinates effectively Implementing productivity standards Resolving operations problems Maintaining reference manuals Administer necessary discipline Coordinates, consults on, and oversees health promotion and health education activities for individuals and groups including; outreach activities in the community; receives and consults on referrals from clinical team members for addressing basic needs for advocacy, reinforces needed education and self-management skills. Performs detailed in-house review and interviews to determine if patient is a viable candidate for community resources/programs, and if so to assist the patient with the application process for any assistance type program. Accurately documents all patient services in the EMR and NextGen as needed for clinical excellence.Oversees monthly Community Health Advisor meetings. Develops written or visual multimedia materials for use with Eligibility events, lectures, or training sessions for presentation to program staff, internal/external clients, community members, or associated outside agencies and organizations. Teaches culturally appropriate, effective health promotion and disease prevention classes and workshops for target population at specific clinic locations. Facilitates training sessions for Matter of Balance and CDSMP, ensuring the successful presentation of information or services to all program clients and event participants. Coordinates and assists with planning and logistics for community events and events at specific clinic locations. Coordinates and supports the evaluation and monitoring of program activities and/or grant-funded programs, and assists in the writing of specifications for evaluation or monitoring of the program(s). Gains and maintains a comprehensive understanding and proficient ability to ensure full compliance of the program with all regulations, standards, or requirements for all Federal, State, and local laws; as well as all standards and requirements for internal/external grants or funding sources. Oversees patient grievance procedures within El Rio Health Supports and participates in the recruitment, orientation, and training of staff and/or volunteers to the various required duties, responsibilities, and assignments within the Eligibility department. Assists and supports in the analysis and prioritization of staff and/or volunteer work assignments that will support successful achievement of program goals and objectives. Provides other duties as necessary which may be assigned by the Eligibility Manager. Maintains a clean, safe, and hygienic work environment in compliance with all Policies and Procedures including but not limited to work areas, workstations, examination rooms, hand washing, infection prevention and control etc. for this position. Demonstrates an understanding of and proficiency with the application of all compliance and reporting requirements respective to Joint Commission Certification (JCC) standards.

Requirements

  • Bachelor’s Degree in Health Care, Social Services; or other related field from an accredited college or university.
  • Three (3) years’ experience coordinating community outreach education and events.
  • Four (4) years ‘experience in Patient advocacy and community resources.
  • Three (3) years of supervisory or managerial experience.
  • If applicable, equivalent combination of education and experience may be considered, and must be directly related to the functions and responsibilities of the job.
  • ACA Certified Application Counselor Certification
  • Level I fingerprint clearance card: current valid and in good standing or have applied for it within seven working days after beginning employment.
  • Employees in this position are required to have reliable transportation that can meet any operational reassignments of the organization during the workday.
  • If an employee is driving during work hours, the employee is required to possess a valid driver’s license and must comply with Arizona vehicle insurance requirements.

Nice To Haves

  • Master’s Degree in Health Care, Social Services; or other related field from and accredited college or university
  • Five (5) years of supervisory or managerial experience.
  • Bilingual (English/Spanish) with the ability to speak, read and write in both languages
