As a Community Health Promoter II, you'll create a positive Scripps Health experience for our patients while being responsible for the following: Primary responsibilities will include: Following up with discharged patients to support social and medical needs Providing community health presentations Assisting with support groups Providing data entry support Overarchingly, this role is responsible for the following: Identifying patients from the emergency department who qualify for Enhanced Case Management Connecting patients to Enhanced Case Management at local partner sites Providing patient support and education which will be offered post discharge to link patients to needed social and medical supportive services Managing documentation as well as maintaining a list of community contacts and referrals Implementing community outreach and facilitate a variety of community-based education Supporting a variety of community-based interventions Assisting in developing educational content
