The Community Health Program Manager II leads the design, implementation, and continuous improvement of programs and initiatives that advance health equity and improve outcomes for priority populations to improve both the managed care performance of the L.A. County safety net and the health of vulnerable L.A. Care members. This position performs a broad range of analytical, planning, engagement, and implementation activities including conducting research and data analysis, solving operational and conceptual problems, building partnerships, and leading program planning, implementation, and monitoring. Program Manager II oversees one or more programs ensuring that the department management and key stakeholders define program goals. This position is responsible for developing program assessment protocols for effectiveness and quality improvement. Program Manager II maintains the program standards of quality and performance. This position oversees the program budget for maximum utilization of resources. This position initiates and develops strong interpersonal relationships with both internal and external business partners to enable shared decision-making and successful program implementation. As part of the Community Health team, the Program Manager helps L.A. Care and its safety net provider partner’s work together to achieve the triple aim of better health quality outcomes, better patient experience of care, and lower cost of care. Program Manager II is accountable for program development, implementation, stakeholder alignment, budget oversight (as assigned), and performance.
