About The Position
Serves as a liaison between patients and the healthcare team, providing support, education, and care coordination services to help patients navigate the healthcare system and achieve their health goals.
Requirements
- High School Diploma or GED required.
- Ability to communicate effectively in the language(s) spoken by the population served.
Nice To Haves
- Completion of specialized short-term training in clinical or medical skills preferred.
Responsibilities
- Participates in the planning and implementation of the plan of care as developed by the Care Manager.
- Reports changes in patient condition (as identified by the patient) to the Care Manager and/or Primary Care Provider, as appropriate, and supports patient and family education.
- Maintains a safe environment for patients, families, and visitors at all times.
- Stays current with Community Health Worker standards, PHS philosophy, and organizational programs to ensure quality care delivery.
- Supports ongoing professional growth by participating in continuing education, including current literature, in-services, and workshops.
- Communicates effectively with patients and customers, providing clear verbal and written instructions and information as needed.
- Participates in local, regional, or state Community Health Worker meetings and events.
- Performs other duties as assigned.
