About The Position

Serves as a liaison between patients and the healthcare team, providing support, education, and care coordination services to help patients navigate the healthcare system and achieve their health goals.

Requirements

  • High School Diploma or GED required.
  • Ability to communicate effectively in the language(s) spoken by the population served.

Nice To Haves

  • Completion of specialized short-term training in clinical or medical skills preferred.

Responsibilities

  • Participates in the planning and implementation of the plan of care as developed by the Care Manager.
  • Reports changes in patient condition (as identified by the patient) to the Care Manager and/or Primary Care Provider, as appropriate, and supports patient and family education.
  • Maintains a safe environment for patients, families, and visitors at all times.
  • Stays current with Community Health Worker standards, PHS philosophy, and organizational programs to ensure quality care delivery.
  • Supports ongoing professional growth by participating in continuing education, including current literature, in-services, and workshops.
  • Communicates effectively with patients and customers, providing clear verbal and written instructions and information as needed.
  • Participates in local, regional, or state Community Health Worker meetings and events.
  • Performs other duties as assigned.
