About The Position

The Community Health Outreach Specialist plays an important role in improving the health and well-being of Mono County residents by connecting individuals and families with essential public health services and resources. Working under the direction of Public Health staff, this position provides case management, health education, program outreach, and client support while helping residents navigate available services. The role also maintains program records, collects required data, and assists with community education efforts. Success in this position requires strong communication skills, cultural sensitivity, compassion, and the ability to build trust with diverse populations across the County.

Experience with case management, customer service, public assistance, or community outreach.
Ability to explain programs, determine eligibility, and connect residents with available resources.

Comfortable working independently while collaborating with healthcare professionals and community partners.
Flexibility, dependability, and willingness to travel to remote communities in varying weather conditions.
Two (2) years of experience that may include: Working with people exhibiting strong communication skills both orally and in writing; Experience presenting information to others or educating others; Counseling and case management; Determining eligibility for a particular program.
Sit for extended periods.
Frequently stand and walk.
Manual dexterity and eye-hand coordination.
Corrected hearing and vision to average range.
Verbal communication.
Use of audio-visual equipment.
Use of office equipment including computers, telephones, calculators, copiers, and FAX.

Nice To Haves

Bilingual skills, particularly the ability to provide verbal and written translation services.
Completion of college courses in public health, health promotion, education, or media related field.
