About The Position

Nak’azdli Health Centre is seeking a Community Health Nurse to join our dynamic and progressive health team. This position provides a comprehensive range of core nursing functions and services in Nak’azdli Whut’en, Fort St. James, BC, within the areas of Community and Public Health. The Community Health Nurse promotes and maintains the health of individuals, families, groups, and the community through culturally safe, trauma-informed, and holistic nursing practice. This role is grounded in the principles of Primary Health Care, with a strong focus on health promotion, disease and injury prevention, population health protection, Prenatal/Postnatal, Family, and Maternal-Child Health.

Requirements

  • Knowledge of, and ability to apply, First Nations cultural principles and protocols in professional practice.
  • Knowledge and application of trans-cultural nursing concepts, including traditional, homeopathic, and other healing practices.
  • Understanding of First Nations cultures and histories, and how culture influences communication, health beliefs, and approaches to care.
  • Knowledge of population health, health inequities, determinants of health and illness, and principles of primary care.
  • Strong foundation in the nursing process and principles of professional nursing practice.
  • Knowledge of evidence-based clinical nursing practice, including assessment, diagnosis, treatment, trauma management, and intervention strategies.

Nice To Haves

  • Bachelor’s Degree in Nursing from a recognized university, or an equivalent combination of education, training, and experience.
  • Minimum one (1) year experience in Community Health or Public Health Nursing.
  • Current practicing registration with the BC College of Nurses & Midwives (BCCNM).
  • Immunization certification.
  • Physical ability to perform the duties of the position.
  • Valid BC Class 5 Driver’s License.
  • Clear Criminal Record Check.

Responsibilities

  • Conduct frequent home visits to provide assessments, case management, health teaching, and direct nursing support to individuals, families, Elders, and clients with chronic or complex health needs.
  • Guide and support the Home & Community Care Program by completing RN-level assessments, assisting with care planning, monitoring client progress, and ensuring services are responsive and client-centered.
  • Work closely with the Community Wellness Department as part of the Health Centre’s wrap-around approach, collaborating on harm reduction, mental wellness outreach, and coordinated client support.
  • Support Prenatal/Postnatal, Family Health, and Maternal-Child Health programs through follow-up care, education, advocacy, and referrals to appropriate resources.
  • Deliver a broad range of public health nursing services, including immunizations, communicable disease control, chronic disease prevention, and health promotion activities.
  • Build strong, respectful relationships with clients, families, Elders, and community members while upholding trauma-informed, culturally safe, and holistic care practices.
  • Maintain accurate documentation and uphold professional nursing standards, confidentiality, and ethical nursing practice.
