About The Position

The Community Health Nurse is a member of the Integrated Health Services team which includes the Health Services and Oasis Clinic and works closely with team members to provide the highest possible quality of healthcare services to clients. The role of the Community Health Nurse is to work with clients to help them achieve their desired health goals.

Requirements

  • Undergraduate degree in nursing from a recognized university or combination of nursing diploma and additional relevant courses/training
  • Registered with the College of Nurses of Ontario (General Class)
  • French (oral expression): Advanced +
  • French (oral comprehension): Advanced +
  • French (reading comprehension): Intermediate
  • Fluency in English, both oral and written
  • Proficiency in the use of computers and various software applications
  • Completing a criminal reference check
  • Verification of educational requirements for this position
  • Linguistic profile to the satisfaction of the Sandy Hill Community Health Centre

Nice To Haves

  • Experience in a primary care setting an asset
  • Three to five years nursing experience in a community setting or combination of community and hospital or public health settings
  • Experience working with a broad range of populations and clients
  • Experience working within a harm reduction approach
  • Experience working in mental health and substance use disorders
  • Knowledge of the impact of social/cultural/economic factors on the determinants of health
  • Strong interpersonal, organizational and problem-solving skills

Responsibilities

  • Meeting the licensing and regulatory requirements of the College of Nurses of Ontario as the professional governing body.
  • Working with clients to help them achieve their desired health goals.
  • Incorporating and strengthening collaborative and interdisciplinary teamwork.
  • Working with people living with mental health, substance use disorder and newcomers to Canada.
  • Working as part of a multi-disciplinary care giving team.
  • Working within the mandate of the Sandy Hill Community Health Centre.
  • Working under stress with multiple demands.
  • Working outside regular hours.
  • Respecting and valuing the diversity of communities and individuals.
  • Being sensitive and alert to signs of a client in crisis.
  • Demonstrating flexibility, good judgment, initiative & creativity.
