About The Position
The Community Health Navigator (CHN) will collaborate with providers on the USF College of Nursing mobile clinic to assist patients with identifying and accessing credible resources related to social needs. The CHN must have a strong passion for decreasing health inequity and assisting patients and families to achieve better health outcomes. The CHN will serve as a resource for education and support for mobile clinic patients and also network and develop relationships with area agencies that provide social support to Tampa Bay residents.
