About The Position

The Community Health Navigator plays a critical role in connecting individuals and families in Cook County with essential healthcare services, with a primary focus on Medicaid redetermination and enrollment. This role includes providing support at local events and community sites, offering one-on-one guidance, and helping community members navigate and access Cook County Health services. This is a contractor role best suited for candidates who are community-oriented, passionate about health equity, and comfortable working independently in public settings. Ideal candidates are organized, adaptable, and able to communicate effectively across diverse populations.

Requirements

  • High school diploma or GED
  • 1–2 years of experience in community outreach, healthcare navigation, enrollment assistance, or a related field
  • Strong interpersonal and communication skills, with cultural humility
  • Ability to work independently and represent the organization professionally in diverse settings
  • Willingness to work evenings and weekends as needed
  • Reliable transportation and ability to travel across Cook County
  • Must bring a personal laptop to each event to access state-run enrollment websites
  • Must have a smartphone for receiving email communications, scheduling updates, and accessing event details

Nice To Haves

  • Lived experience or deep familiarity with the communities served
  • Bilingual or multilingual skills (e.g. Spanish, Polish, Arabic)
  • Familiarity with Medicaid/CHIP enrollment processes or safety-net healthcare settings
  • Basic computer literacy and comfort using data entry systems

Responsibilities

  • Educate community members about Medicaid redetermination timelines and requirements
  • Assist individuals in completing Medicaid redetermination forms and gathering required documentation
  • Serve as a liaison between residents and Cook County Health services, including primary care, behavioral health, specialty care, and preventive services
  • Provide in-person guidance to help community members navigate available resources
  • Attend staff-supported outreach events across Cook County (including evenings and weekends)
  • Build relationships with local partners, including churches, shelters, and grassroots organizations
  • Distribute educational materials and provide enrollment support onsite
  • Maintain accurate records of community interactions and service referrals using simple tracking tools
  • Participate in team meetings, training sessions, and quality improvement efforts
  • Follow standard event procedures, including basic check-in and follow-up communications as outlined during onboarding
