About The Position

Experienced healthcare or public health professional to serve as a Health Navigator, bridging the gap between healthcare services, education, and social services for our diverse community. This role combines community health expertise with advocacy to improve health outcomes and create sustainable healthcare engagement pathways for underserved populations. Requirements

  • Bachelor's degree in public health, nursing, social work, health education, or related field required
  • Minimum 3-5 years of experience in community health, care coordination, or related field
  • Demonstrated experience working with underserved communities
  • Experience in community health education and program development
  • Deep understanding of social determinants of health
  • Demonstrated commitment to health equity
  • Ability to build trust with diverse populations
  • Strong cultural competency and demonstrated excellence in cross-cultural communication
  • Strong organizational and program development skills
  • Project management experience with proven ability to manage multiple initiatives simultaneously
  • Excellence in stakeholder engagement and communication
  • Proficiency in data collection, analysis, and outcomes measurement
  • Valid driver's license and reliable transportation required
  • Ability to lift up to 25 pounds (health education materials, supplies)
  • Schedule flexibility required, including occasional evening and weekend hours for parent programs and community events

Responsibilities

  • Design and implement culturally sensitive health education programs for parents, focusing on preventive care and wellness
  • Facilitate regular health literacy workshops addressing common pediatric health concerns, preventive care measures, and school readiness benchmarks
  • Create accessible educational materials that account for varying literacy levels and cultural backgrounds
  • Establish regular communication channels with parents through multiple modalities (in-person, digital, print)
  • Serve as the primary liaison between families and the healthcare system
  • Identify and address barriers to healthcare access, including transportation, scheduling, insurance, and communication challenges
  • Navigate insurance enrollment processes and connect families with financial assistance programs
  • Develop strategies to improve healthcare utilization rates among enrolled families
  • Coordinate with local healthcare providers and social services to establish comprehensive care networks
  • Improve time to compliance for health requirements (immunizations, physicals, dental care)
  • Coordinate school-based health services including health screenings, immunization drives, and mental health referrals
  • Conduct regular needs assessments to identify healthcare gaps and barriers
  • Develop and implement strategic plans to address identified healthcare challenges
  • Track and analyze program metrics to measure impact and outcomes
  • Prepare regular reports on program effectiveness and community health trends
  • Build upon and expand existing health professions pipeline framework for high school students
  • Develop detailed curriculum, programming structure, and sustainability plan for career exploration activities
  • Coordinate shadowing opportunities and mentorship connections with healthcare professionals
  • Create and strengthen partnerships with local health professional schools and healthcare sites that offer training programs to help high school students achieve credentials
  • Establish metrics and tracking systems for student engagement and post-secondary health career enrollment
  • Identify funding opportunities to support program growth and student participation
  • Build partnerships with local healthcare organizations and community resources
  • Manage and allocate health education resources effectively
  • Support grant applications for healthcare-related initiatives and maintain compliance with grant reporting requirements
