Since 1970, GPHA has expanded to become one of the premier providers of primary care, specialty care, dental services, and behavioral healthcare in the Greater Philadelphia area. We are presently seeking full-time Community Health Navigators. Community Health Navigators enhance the quality and utilization of HIV services by providing counseling, outreach, and referral services for patients and their support systems.

Requirements

• High school diploma
• Ability to travel to multiple sites throughout GPHA's healthcare network
• Ability to work proactively both independently and as part of a team

Nice To Haves

• At least one to two years’ experience working with HIV or OB patients
• Experience in integrated health services
• Experience in social determinants of health assessment
• Experience in outreach
• Experience in caseload management
• Experience in tracking

Responsibilities
