About The Position

Community Health and Programs Manager (Full-time; 40 hours/week) Generations Family Health Center, Inc. is a regional community health center covering a thirty-seven town rural region in eastern Connecticut with sites located in Willimantic, Danielson, Norwich and Putnam. Our dedicated staff provides a full range of primary care services to all ages including medical, dental and behavioral health care, as well as care coordination and community outreach. In addition to our main facilities, we offer mobile programs and School Based Health Centers. LOCATION: Willimantic, CT JOB SUMMARY: The Community Health Programs Manager provides operational oversight of the organization’s Community Health Worker (CHW) program and coordinates the execution of community-based initiatives that support patient engagement, social determinant of health (SDOH) navigation, and community access to care. The Manager ensures that staff activities, Mobile Health Unit deployment, and community engagement efforts are coordinated, measurable, and aligned with the organization’s strategic priorities. The role establishes structured workflows, productivity expectations, and reporting mechanisms to strengthen visibility into community health programs and their impact on patient access and health outcomes. The Manager also represents the organization in relevant community coalitions, advisory groups, and collaborative initiatives to strengthen partnerships, support coordinated community health strategies, and maintain awareness of emerging community needs. The Manager supervises staff, coordinates operational components of grant-supported community programs, and supports collaboration across clinical, operational, informatics, and community partner teams. The role also supports structured reporting related to community engagement, SDOH trends, and program outcomes. Reporting to the Chief Operating Officer, the Manager is accountable for the performance, coordination, and outcomes of community health programs, outreach activities, Mobile Health Unit operations, and retains oversight and accountability for program effectiveness, operational alignment, and measurable outcomes.

Requirements

  • Minimum of four years of experience in community health programs, care coordination, outreach programs, or related healthcare or social service environments is required.
  • Demonstrated experience coordinating community-based programs and working with multidisciplinary healthcare teams, and working in community health centers, public health programs, or organizations serving underserved populations is required.

Nice To Haves

  • Bachelor’s degree or higher level of education is strongly preferred.
  • Supervisory experience is strongly preferred.
