About The Position

The Community Habilitation Counselor position involves providing training and supervision to individuals, incorporating the use of community resources. The role requires ongoing communication with the Community Habilitation Coordinator and attendance at scheduled meetings. Shifts are part-time, ranging from 4-16 hours per week, typically in the late afternoon to early evening during the week, with some hours on Saturday or Sunday, across Queens, Nassau, and Suffolk Counties. Requirements

  • Valid NY State or out of state driver's license
  • HS diploma/GED
  • Own vehicle to use while working the case (required for most cases)

Responsibilities

  • Participation in the training and supervision if the individual while incorporating the use of community resources.
  • Maintain on-going communication with the Community Habilitation Coordinator and attend scheduled meetings for discussions when necessary.
