The Community Guide will engage and work alongside clients from diverse ethnic backgrounds, focusing on those with complex social needs and chronic health conditions. This role involves reaching out to members to enroll them in Reema Health's care model and providing high-quality, meaningful support to help them achieve improved health and wellbeing outcomes. The Community Guide will assess member needs, including social determinants of health, to create member-centered care plans, establish trusting relationships, and coordinate referrals and appointments with various healthcare providers. A key responsibility is to independently manage a caseload of 60-100 members through cold outreach, while also providing coverage for other Community Guides when needed. The role requires active listening, empathy, maintaining professional boundaries, and confidentiality. Community Guides will visit members in person as appropriate, utilizing various communication methods (text, phone, email, face-to-face) to meet member needs and connect them with community and health resources. They will also prepare members for service endings by focusing on self-sufficiency and independence. Accurate and timely documentation using the Reema App, understanding HIPAA compliance, and maintaining thorough member records are essential. The role requires working independently and collaboratively in a fast-paced startup environment, adapting to change, and serving diverse members with dignity and respect. The Community Guide will also build relationships with community resources and customers, and partner with colleagues across the team and company.
