About The Position

This role provides direct leadership and performance oversight for Community Educators, the Community Educator Trainer, and Clinical Hospital Liaisons. The position establishes performance expectations, monitors referral and activity goals, and conducts regular one-on-ones, performance reviews, field ride-alongs, and coaching. It also supports recruitment, onboarding, development, and retention of team members, and reviews schedules, timecards, and PTO. The role leads daily sales and referral development activities, establishes growth expectations, and monitors performance using data to identify opportunities and gaps. Action plans are developed and implemented to improve performance and achieve growth goals, with an active field presence maintained through account visits, ride-alongs, coaching, and relationship development. Consistent CRM utilization and documentation of sales activity are promoted and monitored. The position also focuses on building and maintaining relationships with physicians, hospitals, skilled nursing facilities, assisted living communities, home health agencies, and other referral partners, identifying new referral opportunities, and developing strategies to grow existing accounts. It supports team members in strengthening key referral relationships and represents the organization at professional, community, and networking events, promoting organizational services and programs. Team development includes coaching in relationship development, consultative selling, account management, communication, and customer service, partnering with the Community Educator Trainer for onboarding and training. High performance is recognized, and professional growth and accountability are promoted. Operational collaboration with the Access & Growth Manager is key for executing departmental growth strategies, and collaboration with Admissions & Intake, Clinical Operations, Marketing, and other departments ensures timely patient access and a positive referral partner experience. Compliance with organizational policies, regulatory requirements, and ethical marketing practices is ensured. Additional duties are performed as assigned.

Requirements

  • Minimum of five (3) years of experience in healthcare business development, sales, referral development, community outreach, account management, or a related field.
  • Minimum of two (1) years of leadership or supervisory experience.
  • Demonstrated success achieving sales, referral, growth, or business development goals.
  • Experience building and maintaining professional relationships with healthcare providers, hospitals, physicians, skilled nursing facilities, assisted living communities, and other referral sources.
  • Strong leadership, coaching, performance management, communication, and relationship-building skills.
  • Experience using CRM systems and analyzing sales, referral, activity, and market data.
  • Valid driver's license, acceptable driving record, and reliable transportation.
  • Ability to travel regularly throughout the assigned service area.

Nice To Haves

  • Bachelor's degree in Business, Healthcare Administration, Marketing, Communications, Public Health, or a related field preferred; equivalent combinations of education and relevant experience may be considered.
  • Experience in hospice, home health, post-acute care, or another healthcare-related industry.
  • Experience managing a team responsible for healthcare sales, referral development, community education, or business development.
  • Bachelor's degree in a related field.
  • Knowledge of healthcare regulations, referral practices, ethical marketing standards, and the continuum of care.

Responsibilities

  • Addresses performance concerns and partners with Human Resources as appropriate.
  • Supports recruitment, onboarding, development, and retention of team members.
  • Reviews schedules, timecards, PTO, and other supervisory responsibilities.
  • Leads daily sales and referral development activities across assigned markets and hospital accounts.
  • Establishes referral and growth expectations and monitors individual and territory performance.
  • Uses referral, activity, and market data to identify opportunities and performance gaps.
  • Develops and implements action plans to improve performance and achieve growth goals.
  • Maintains an active field presence through account visits, ride-along, coaching, and relationship development.
  • Promotes and monitors for the consistent CRM utilization and documentation of sales activity.
  • Builds and maintains relationships with physicians, hospitals, skilled nursing facilities, assisted living communities, home health agencies, and other referral partners.
  • Identifies new referral opportunities and develops strategies to grow existing accounts.
  • Supports Community Educators and Clinical Hospital Liaisons in strengthening key referral relationships.
  • Represents the organization at professional, community, and networking events.
  • Promotes organizational services and programs across the continuum of care.
  • Coaches team members in relationship development, consultative selling, account management, communication, and customer service.
  • Partners with the Community Educator Trainer to support onboarding, competency development, and ongoing field training.
