The Community Disability Worker role involves supporting adults, youth, and children with developmental and physical disabilities. This position requires implementing individual service plans, providing direct care, and assisting individuals in achieving their personal goals for development, community participation, and inclusion. The role also involves promoting safe environments, maintaining records, and collaborating with individuals, families, staff, and community representatives. Employees are expected to be aware of community issues, model appropriate behaviors, and assist individuals in developing life skills and maintaining their home living environments. Direct care for high-needs or medically fragile individuals may also be required.
