This role involves providing awake night support on Fridays and Saturdays, from 10:30 PM to 9:00 AM, every other weekend. The Community Disability Worker will implement individual service plans, provide direct care, and assist individuals in participating meaningfully in their personal lives, community, and personal growth. This includes supporting individuals in developing relationships, participating in activities, and achieving personal goals. The position requires maintaining records, communicating professionally, and implementing recommendations from supervisors. The role also involves identifying and resolving issues, pursuing personal skill improvement, and participating in team meetings.
