About The Position

Fox Trail Memory Care is seeking a Community Director - LPN to lead both the clinical and operational success of their memory care home. This role involves guiding and supporting care staff, managing schedules, overseeing resident assessments and medication administration, ensuring regulatory compliance, and serving as a key point of contact for families and healthcare providers. The Community Director will also lead community events, partner with marketing to maintain occupancy, and foster a welcoming environment for residents and staff. The company emphasizes a small, home-like community setting where residents feel safe, supported, and valued.

Requirements

• Active LPN license (New Jersey required)
• 1+ year of experience in senior living, assisted living, memory care, or home care
• Prior leadership or management experience in a healthcare setting
• Strong knowledge of dementia and Alzheimer’s care
• Excellent communication, organization, and leadership skills
• Comfortable with Microsoft Office and learning new systems

Nice To Haves

• Bilingual (Spanish) is a plus, but not required

Responsibilities

• Guide, coach, and support care staff (CMAs, CHHAs, and activities team)
• Hire, train, and develop a strong, reliable team
• Manage staffing schedules to ensure consistent coverage
• Complete resident assessments (admissions, quarterly, and as needed)
• Oversee medication administration and clinical documentation
• Ensure compliance with NJ DOH regulations and company standards
• Conduct audits, quality checks, and maintain accurate records
• Serve as the main point of contact for families and healthcare providers
• Lead engaging community events and support daily programming
• Partner with marketing to maintain occupancy and lead tours
• Create a welcoming, well-run home environment for residents and staff
