About The Position

The Community Coordinator Team Lead is responsible for providing a mix of direct service and administrative/supervisory support at LMM’s Men’s Shelter. The goals of the position are to: 1) ensure homelessness is brief, rare, and a one-time experience through directly providing high quality and responsive case management for people exiting homelessness; and 2) refine and improve internal and external communication, processes, data/tracking, and outcomes through providing peer leadership and oversight. The Community Coordinator Team Lead will have excellent time management and self-motivation skills, experience providing case management in homeless services, and a demonstrated interest in or aptitude for program monitoring and management.

Requirements

  • High School Diploma or GED
  • Proven experience identifying, accessing, and coordinating with community partners and social service resources.

Nice To Haves

  • Bachelor’s Degree preferred
  • Experience working with people experiencing homelessness, veterans, people with substance use disorders, people with re-entry backgrounds, and/or other vulnerable populations, preferred.

Responsibilities

  • Support clients in the formulation of Exit Plans that will move participants toward rapid exit of homelessness and ensure linkage with most appropriate destination the client is eligible
  • Link and refer clients with resources and to community partners including: obtaining identification, benefits and entitlements, mental health and substance use disorder services, housing interventions, as appropriate with brief stay in Community Link and refer clients to other Communities within Shelter as appropriate to clients’ Exit Plans
  • Ensure linkage to Cuyahoga County’s Coordinated Entry, if not assessed prior to Shelter entry
  • Assist in arranging transportation to critical destinations (e.g. housing, medical, employment)
  • Float between Communities as needed
  • Maintain participant data and statistical information, client database records, Housing Assessments, and progress notes in HOPE
  • Actively participate in quality improvement activities of the program/agency
  • Conduct dormitory and living area inspections on a regular basis
  • Assist in distribution of clothing and hygiene items as available to program participants
