Every day, you’ll use your knowledge, empathy, and organizational support to help people in need access the resources they require to succeed.

Requirements

• 1 to 3 years of experience in case management services with veterans, mental health, substance abuse, HIV/AIDS, or homeless clients
• Compassionate and patient listener
• Ability to juggle multiple tasks
• Problem-solver
• Organized and detail-oriented
• Previous experience as a Case Manager or in social services
• Bachelor’s degree in Social Work, Psychology, or a related field preferred

Responsibilities

• Be the first point of contact and build trust with clients facing complex life situations.
• Develop individualized service plans to guide clients toward achieving their goals.
• Connect clients with community resources, services, and support networks.
• Advocate on behalf of clients for housing, healthcare, financial assistance, and more.
• Track and document progress, celebrating victories big and small!
• Collaborate with a passionate team of colleagues and partners to create lasting impact.
