About The Position

Brighter Heights Maine is seeking a Community Case Manager for Adults with Developmental Disabilities and Autism to join their program. This is a hybrid position based in Bangor. Initially, the role requires working out of the Bangor office for training and support, after which it allows for flexibility in creating your own schedule to see clients in the greater Bangor area (within approximately 45 minutes). The agency emphasizes teamwork, self-care, and a Trauma Informed approach, with supervision and support available to promote employee success and well-being.

Requirements

  • Strong time management and organizational skills.
  • Excellent verbal and written communication skills.
  • Experience in working with people with developmental disabilities or a related human service field.
  • 2 years of prior case management experience preferred.
  • Strong computer skills, ability to learn an EMR.
  • Bachelor's degree from an accredited institution.
  • Must have a vehicle, provide proof of license, insurance, and registration.

Responsibilities

  • Work out of the office for the initial few months for training, skill development, and caseload growth, with remote work available once necessary skills are demonstrated.
  • Complete State of Maine and Brighter Heights Maine required trainings.
  • Provide own transportation for meetings, home visits, and community visits (must have a vehicle, license, insurance, and registration).
  • Oversee, monitor, and coordinate all needed services for individuals on the caseload.
  • Communicate effectively with individuals and their teams.
  • Make referrals, coordinate services, and obtain funding for needed services.
  • Develop, monitor, report, and assure annual Person Centered Plans address all needs and desires of the individuals supported.
  • Document effectively and in a timely manner in the State of Maine Evergreen System.
  • Manage a caseload of 20-23 individuals.
  • Meet billing expectations weekly.
  • Participate in monthly supervisions.
  • Communicate with supervisor as needed for support, problem-solving, and understanding processes/procedures.
  • Maintain professional and ethical boundaries.
