Community Care Worker IV - Serves as an advanced worker in a field unit providing direct delivery and contract services to Texans in need Community Care Services. Work involves conducting home visits, interviewing individuals who are older or have a disability, documenting information, determining need for services and developing service plans; verifying data, explaining program benefits and requirements, referring individuals for appropriate services, and assuring authorizations are registered in automated systems. Authorizes and monitors Community Care Services Eligibility (CCSE) services to determine if services are meeting the individual's needs. Additional duties may include serving as acting supervisor in the absence of the unit supervisor, reading cases, providing assistance and policy interpretation for other staff, training new workers or completing special reports. Work includes operating a computer. May work on special projects for the region. Maintains a positive and productive attitude while working in a fast-paced environment. Requirements

Current Valid Driver’s License.
Knowledge of gerontology, the dynamics of the aging process, and special problems of Long-Term Care Services individuals.
Knowledge of interviewing techniques to obtain personal information, make inquiries and resolve conflicting statements.
Ability to effectively communicate orally and in writing.
Knowledge of the English language such as vocabulary, grammar and organization of expression, composing correspondence, and reporting casework.
Ability to listen to individuals, exploring and reflecting feelings, and present relevant alternatives describing fees, bills and consequences of choices.
Ability to complete work within required timeframes.
Ability to appraise living conditions, including physical and emotional environment.
Ability to establish priorities according to relative importance and deadlines, set goals and manage time efficiently.
Ability to monitor unit operations.
Ability to observe employee performance and record observations.
Ability to establish rapport with individuals receiving services, co-workers, contract agency staff, and staff from other federal or state agencies.
Ability to communicate with community members to request assistance for individuals, promote support for the individual or coordinate resources and programs.
Ability to read cases effectively and accurately.
Ability to communicate policies and procedures to staff.
Ability to operate computer and general office equipment.
Knowledge of resources that serve individuals who are older or have a disability.
Must have reliable transportation.
Must be able to attend training which may require overnight stay.

Nice To Haves

Prefer at least two current years’ experience with Community Care Services.
Must be able to travel a minimum of 80%
Excellent written communication as observed by detail and completeness of information provided on the state application
Prefer experience mentoring others
Prefer experience serving as an acting supervisor

Responsibilities

Authorizes and monitors CCSE program services. (20%)
Determines eligibility for CCSE program services. (15%)
Provides policy and procedural guidance to staff to include training and/or mentoring. (15%)
Documents information manually and into multiple automated systems. (10%)
Verifies case information utilizing multiple automated systems. (10%)
Completes special assignments and/or reports. (10%)
Assists the Supervisor in Unit administration. (10%)
Refers individuals to other programs and/or agencies. (5%)
Reviews and responds to legislative and non-legislative inquiries. (5%)
Attends work on a regular and predictable schedule in accordance with agency leave policy and performs other duties as assigned.
Be familiar with and follow all HHS policies and procedures relating to job performance and work rules.
