About The Position

This fixed-term post will provide additional capacity while the service continues to grow and develop. You will work closely with Shared Lives carers, citizens, families and social work colleagues to review placements, update support plans, respond to issues, and help ensure people receive safe, personalised support. Be part of a supportive service where relationships, good matching and person-centred practice are at the heart of what we do. Shared Lives provides family-based care for citizens with a range of needs. The team assesses and supports carers, oversees placements, and works with colleagues across adult social care to help people achieve positive outcomes.

Requirements

  • Experience or understanding of adult social care and working with people who have a range of care and support needs.
  • Well-organised and motivated person who can manage their workload and communicate clearly.
  • Uses initiative, works well with others and can respond calmly and creatively to challenges.
  • Experience working with adults in a social care/health/voluntary sector setting.
  • Experience developing and reviewing personalised care or support plans.
  • Ability to keep clear, accurate records and evidence decisions appropriately.
  • Confidence managing a caseload, prioritising tasks and working independently.
  • Ability to build positive working relationships with carers, colleagues and partner agencies.
  • Ability to speak fluent English (essential requirement).
  • A DBS enhanced check for a regulated activity is required for this post.
