We are seeking an experienced Home Health Community Liaison who thrives on relationship-building, enjoys educating referral partners, and is energized by driving growth through strategic networking.

Requirements

  • 1+ years of Home Health marketing experience or healthcare sales experience
  • Established referral relationships within the Dallas market
  • Knowledge of Medicare guidelines, HIPAA, and compliance standards
  • Exceptional communication and relationship-building skills
  • Ability to work independently and achieve performance goals
  • Valid driver's license, reliable transportation, and auto insurance

Nice To Haves

  • Experience marketing to SNFs, ALFs, physician offices, and hospitals
  • Proven record of achieving referral growth targets
  • Experience using CRM or EMR systems

Responsibilities

  • Build and maintain relationships with physicians, hospitals, skilled nursing facilities, assisted living communities, group homes, and case managers
  • Develop and execute territory growth strategies
  • Conduct in-person visits, educational presentations, and follow-up meetings
  • Attend community events and professional networking opportunities
  • Collaborate closely with our clinical team to ensure smooth patient transitions
  • Maintain accurate records of activities, referrals, and outcomes
  • Identify market opportunities and provide feedback to leadership
